Gee, a little preplanning and shifting before the hill, in other words, a wee
bit of finesse will get you up there. Or just sit down, shift and stand back
up. In a race it might be important, but just ridin along, no big deal.
Remember even in 1935 or so, before genuine rear ders, the two rod 'der' from
Campagnolo was usable, even on hills. With enough practice, ya can do anything.
Sure, you can. However, it's not as convenient or as fast as index
shifting, most importantly the STI or Campy brake lever integrated
shifters.
It's not all that often I shift when standing, but there are times it's
certainly necessary in racing moments. In those cases the 10-15 meters
you save by not sitting, shifting, re-standing, and sprinting can make
a huge difference.
