In France, all racers must get a doctor's certificate at periodic
intervals. The doctor that signs it must be certified for "Medicine
du Sport".
Some (if not all the clubs) also request a similar certificate for
regular admission. But here they also accept certificates issued
by a family doctor (a generalist).
So, what the e-mail means is that you must present either the license
(that implies you had the medical test) or that you take the test
yourself at any generalist.
All this is required for legal and ensurance coverage.

It means you will be considered as Randonneur. In other words: you will
run on your own. What this translate in practice I cannot tell, it varies
from tour to tour.
I would try to get the certificate. There are no dedicated tests required.
All the doctor must say is that there are no counter-indications for you
practicing biking.
Such certificates are usually accepted up to 12 months after the issuing
date.
