LDL Levels ?

  1. Despite very good cholesterol levels my doctor
    prescribed Zocor 10mg . TC 178 , HDL 40 , LDL 97 .
    According to him the guidelines for diabetics is
    to maintain LDL at below 70 . Not very convincing .

    When you apply the Friedewald formula to your posted numbers:

    TC = LDL + HDL + (Triglycerides/5)

    it suggests that your triglycerides are about 200 mg/dL

    That's way too high. You need to work on those numbers ASAP.

    An important goal for a diabetic: a triglycerides/HDL ratio less than 3.0

    Yours is over 5. That leads to high risk of premature heart attack.

    You also want a Total Cholesterol/HDL ratio less than 4. Your 4.5 ratio is
    close, but still too high for a diabetic.

    Your cholesterol "looks" like it's better than mine (my TC = 184) but your
    ratios are off. My triglycerides/HDL ratio is 1.1; my Total/HDL ratio is
    2.7

    many from which to choose."
    That being said, here is a short summary of the current situation:

    For some time, the standard guideline for initial treatment of
    cholesterol in diabetics was LDL>100. Having diabetes is considered a
    risk factor for adverse cardiovascular events equal to having had a
    prior heart attack. Research was published showing that the
    cardiovascular protection of lower cholesterol continues to increase
    as LDL is decreased past 100. Research was also published which
    showed, at least some, statins have a cardiovascular protective effect
    above and beyond their cholesterol lowering properties. This lead to
    pressure to treat cholesterol in diabetics before they reached the
    LDL>100 guideline and to use statins as the primary treatment choice.
    Statins do have serious side effects in a small percentage of
    diabetics. 1/3 of first heart attacks are fatal in the general
    population and it is more in the diabetic population. There is a
    general consensus among physicians that the trade off comes down on
    the side of statin treatment, generally after dietary modification has
    failed. Some very aggressive physicians suggest statin treatment in
    all diabetics independent of cholesterol level. There has been an
    expectation that the guidelines for initial treatment would be
    modified. I hadn't heard that it had happened yet, but it is not
    surprising.

  4. Forget the nonsense. Just don't allow your cholesterol to get
    oxidized, as even Dr. Richard Stein of the AHA is now saying. Best
    cholesterol levels are in the 200-220 range. Very low LDL levels mean
    very high risk of cancer. High TGs reflect diet, and mean you are
    probably eating too many carbs and not enough high quality protein and
    saturated fatty acids. Again, the key is not to allow them to get
    oxidized.

    What are your triglycerides, and were you fasting for at least 12 hours
    before the test?

    I'm diabetic, my numbers are a good deal higher than yours, but my
    ratios are good, my bg is tightly controlled, and I wouldn't take a
    statin on a dare.

    The purpose of minimizing your triglycerides/HDL ratio is to prevent
    cholesterol particles from being oxidized.

    You have borderline LDL, even for a non-diabetic. You have very high
    triglycerides. You have low HDL. Combined with your diabetes, those things
    put you at extremely high risk.

    You might be able to bring your risk down to an acceptable level with
    exercise, weight loss, and diet (low carb and low saturated fat), especially
    if you use every trick to increase HDL.

    The culprit is "Omental" fat, the fat underneath the abdominal
    muscle layer. I have seen a medical article which commented on the fact
    that folks from India can exhibit unhealthy levels of Insulin Resistance
    despite appearing distinctly non-obese by European/African standards. They
    seem to have more "active" Omental fat so a little bit goes a long way.

    Fish oil is great, but I don't know of a single user of policosanol
    whose gotten good results, like the ones I got from pantethine.

    At 450 mg, twice per day, changing nothing else, my LDL decreased 70
    pts., my HDL went from 58 (had dropped from 68) to 70.

    My TGL, IIRC, was in the 120s, but should be lower now, with better bg
    control, diet, and metformin added.

    Great Susan , did you have any side effects ?
    It is not available in walgreens and other drug chains ,
    may be I need to try at vitamin stores .
