Quality Bike Products lists two or three manufacturers:

which means that any bike shop in the US should be able to get one for
you. Talk to the local BMX dudes- they'll know where to get them.

BMX cranks are much stronger and better-looking than MTB and road
cranks; why don't you just use one of those? Double- and triple-ring
spiders for them are readily available:

The BMX sector is in the midst of a moronic infatuation with threaded
BBs at the moment. As a result, "Euro" BB bearing assemblies for BMX
cranks are a lot more common (and a lot more useful) than the type of
adaptor you describe. BMX and freestyle riders evidently haven't yet
figured out that threaded BB shells suck, but they long ago discovered
that square taper spindles are suitable only for child riders.

Anyway, here's what you are looking for. If you ask me, it's an
expression of exceedingly poor taste to use one. But go right ahead if
you must:
