Broken/bent axle? Broken/bent freehub "fixing bolt" (the hollow bolt
that attaches the freehub to hub). I have bent axles and broken fixing
bolts.

This is symptomatic of not having the correct number of bearing balls
in the hub. Per Sheldon, "Most rear hubs use 9 1/4" balls on each
side".

You may have one too many or one too few balls on the left side.

Thanks, I think that was it. I bought the hub on ebay a few months
ago. It have never been laced and I assumed it was new. When I took
the axle out earlier this afternoon to check for a bend I noticed that
the hub was packed full of what looked like Phil green grease.
Unfortunately I didn't count how many balls came out of each side,
only that there were 18 total. I just put it back together now, and
it seems good. I am thinking that the previous owner took the hub
apart to fill it with Phil grease then put it back together with 11
balls on one side and 9 on the other. If I wasn't so lazy, I would
take it back apart to see if it is possible to do that.

Balls out of place on the left side. When you opened the
hub to lube it before putting it in service, the balls
tumbled to one side.

ITYM 10 on one side and 8 on the other. It's an easy mistake to make
on some hubs. (On some, 10 will simply not fit.)

Yes. 8 and 10. I may never know if it was that, or if the balls had
just shifted, as A. Muzi suggests. I am still sort of surprised how
the hub went from seemingly normal while building the wheel, to all
crooked and wobbly, without taking it apart. I had assumed that an
incorrectly assembled hub would be immediately apparant. I definately
learned something.
