I can't get into top gear. The rear derailleur goes as far as 5th out of 6 cogs but won't go no further.
I've spent a couple of hours following the advice at http://www.parktool.com/repair_help/FAQrindx.shtml but I can't fix it. I sit the bike upside down, set the limit screw, change gear by pulling on the cable manually, it moves out nicely into top gear. Then I get on the road cycle up and down and it won't go any higher than the 5th cog.
When it is in top gear the cable is nice and loose but afaics there must be something stopping it getting out to the 6th cog. I'm pretty sure the limit screw is not the problem. I've tried a hopeful squirt or two of lubricant at various pivot points. Am I missing something obvious or does it sound like it needs a good clean?

Clean the under-bottom bracket cable guide, if you have one. If nothing else works, lubricate or replace the whole gear cable (inner & outer).

This happened to me after a crash when it turned out to be a slightly bent hanger. Carefully try bending the hanger away from the frame very slightly, this might do it. If this doesn't work fit new inner and outer cables.

Yes, thoroughly clean the derailleur and the chain .. and re-lube with light oil .. 😉
I generally use Gunk to de-grease a chain that's particularly mucky ..
Sounds to me, frankly, as if your lever isn't pulling the cable far enough to move the mech out to the sixth cog, in which case you may want to pull a bit more cable through at the anchor screw. Obviously, screw the cable adjuster(s) right in before you do this, and reset them afterwards.

Normal mechs shift to smaller sprockets by releasing cable not pulling it, the spring in the mech doing the work. Only "Rapid Rise" type mechs work the other way round.

I'm glad I'm not the only one gets this messed up. It's like the top string on a guitar. Do you mean top as in furthest-from-the-ground (assuming you're standing up) or do you mean top as in 'ping' instead of 'twang'.
And thanks for the suggestions so far. I have tried cleaning the derailleur mechanism (OK only from the outside) by painting this stuff on called Jizer (I'm not kidding) which once managed to clean six months of road dirt off a motorbike I used to have. It had no effect.
