I don't find it surprising at all. Spurlock basically doubled his
caloric intake from ~2500 cal to ~5000 cal/day (oink, oink), avoided
exercise, and ate every single meal there.
Spurlock certainly could have continued to walk instead of taking
cabs.

His reduction in exercise was the MORE likely culprit of many of his
woes than his WOE. The guy was wlaking upwards of ten miles a day then
he stopped cold turkey and started cabbing it...seems very few people
pick up on this.
That too was part of the design of his experiment. Most Americans walk less
than 5000 steps per day, so he wore a pedometer and restricted his walking
to that much.
He wanted to see, and document, the health effects of eating "typical" meals
at McD's, and exercising no more than the average couch potato. The fact
that it had such a huge impact on his health, in such a short period of
time, was pretty interesting.
Was it a documentary with a distinct point of view? Sure, most
documentaries are. But, it was also very well done, and made it's point.
And, McD's ended their "Super Size" offers even before the movie came out,
so it achieved some good too.
