  1. Here's one for the paranoid but I recently heard that there might be a link with prostrate cancer and cycling. Just wondered what the consensus is on this and what people do to offset concerns over this.

    The answer would seem to be no (prostate specific antigen is the prime
    prostate cancer marker in blood)

    One of the standard initial tests for prostate cancer is PSA (prostate
    specific antigen) in the blood. The other is physical examination.
    If these give a positive then a tissue biopsy may follow - (just tiny

    for the patient but may cost the taxpayer a few hundred pounds
    unnecessarily.

    As I said, the evidence is mainly anecdotal, although it can be
    confirmed loosely from experience.* Staying out of the saddle,
    or adapting the riding style for a minimum of four weeks prior
    to a sampling for PSA should be sufficient to cancel out any
    elevating effect if it exists. Although ISTR one doctor gave
    me a figure closer to eight weeks. Although whether he was
    being ironic, in the face of my fund of recently acquired anecdotal
    knowledge, I wasn't too sure at the time.

    Over the age of 50, PSA readings can give up to 75%-80% false
    positives, so its worth avoiding unnecessary biopsies which
    might be ordered as a result of a misleading high reading.

    ISTR There may be some evidence that cycling can lead to an
    enlarged prostate but this isn't dangerous in itself, although
    maybe inconvenient.

    See the list of research papers I posted above which show your anecdotal
    evidence is wrong. Cycling does not lead to elevated PSA levels. As
    for prostate cancer then cyclists do get it but then again since one in
    three men over 50 will have some traces of prostate cancer its hardly
    surprising.

    Badly designed bicycle saddles can cause damage to the plumbing in your
    groin, leading to all sorts of nasty things like erectile disfunction
    and, allegedly, an increased risk of prostate cancer. Women suffer
    slightly different but equally unpleasant damage from poorly fitting
    saddles.

    The problem is pressure on soft parts of your anatomy that aren't
    designed to take pressure. The solution is a saddle which supports you
    under your ischeal tuberosities or 'sit bones', the big boney lumps on
    the back of your pelvis, and /nowhere/ /else/. What this means is that
    the saddle has to be very firm indeed, and the right width for you.
    Different people differ, and a fat person doesn't necessary have wider
    spaced sit bones than a thin person. 'Specialized' dealers now have a
    device to measure the width of your sitbones, and it really works.

    FWIW the saddles I'd recommend are Brooks leather saddles, which are very
    firm, very comfortable, but, alas, somewhat heavy; or the Selle Italia
    SLR series (apart from the XP 'extra padding' model) which are very
    firm, very comfortable, and extremely light.

    Do not make the mistake of using a 'gel' or padded saddle. Your sit bones
    sink in, and your weight is distributed over your whole groin area.
    Initially this feels comfortable, and if you only ever do occasional
    short rides it may be OK. But in the long term it's going to injure you.

    I presume the papers quoted sample sizes, and the respective time
    intervals between the actual sampling and the last time in the

    Have there been any studies on such questions, do you know?

    be able to provide the comprehensive answers that would seem to
    be required.

    That's not what's in question.

    It's maybe just as well nobody is likely to take your citations
    of the earlier papers, regarding elevated PSA among cyclists on trust
    then, isn't it ?

    The question under consideration, is what proportion of male cyclists,
    on autopsy at any age, are shown as having had prostate cancer at 50,
    and at what stage, as compared with the general male population at 50
    as subsequently examined on autopsy.

    As cycling behaviour doesn't normally feature in medical records
    except possibly in repect of injuries, the difficulties in compliling
    such information shouldn't need too much explanation.

    For most people, at least.

    Posting URL's is pointless. Most journals are only available on-line if
    you have a subscription. If you do its easy enough to look them up, if

    Since it is blood circulation, the levels will not drop rapidly after
    stopping cycling so the timing is not particularly important other than
    if the release is as a result of cycling it needs time to get into the
    blood stream. Since some of the rides are up to 250 miles though its
    unlikely that there will be significant change during the first hour or
    so after stopping.

    However, at age 50 approx one third of males will have evidence of
    prostate cancer cells and one half will have BPH. By age 80 its
    virtually 100% for cancer and 80% for BPH. I am not aware of any
    studies looking for an excess of prostate cancer in cyclists but it
    would have to be a pretty massive study to show one given the above
    prevalence.

    Now do you have any evidence to support your suggestion that there is an
    excess of prostate cancer in male cyclists or is it just speculation
    based on some cyclists getting prostate cancer?

    No it wasn't. I was citing the O.P's question about the relation
    between prostate cancer and cycling. For which, as I pointed out
    there is very little evidence.

    The point being that in general cyclists are don't figure very highly
    in experimental modelling. As they're not thought that significant
    in statistical terms.

    I did however produce some anecdotal evidence coupled with my
    own personal experience, which again is supported by some studies
    into the affect of exercise in general, and cycling in my own
    particular case, immediately prior to giving a blood sample,
    and the effect this may have on elevating PSA levels.

    And I hope my own experience may be useful to other cyclists. And
    their doctors. In not taking an isolated PSA reading, in the absence
    of any other indicators as constituting a good reason for having a
    biopsy before another PSA reading following midified behaviour.

    That, if you could actually be bothered to read my first post, or
    had the comprehension skills to understand what I wrote, was my
    actual point.

    is pointless.

    Tony has tried to 'post' a few detailed replies to the original and my
    posts only to be 'blasted' by others (all part of newsgroup life I
    know, and there was me thinking they are a good place for helping each
    other)

    It would 'appear' that there is no link between long distance saddle
    use and prostate cancer which is good enough for me at 55 this year. I
    will still have a chat with my consultant mate though (only problem is
    I know I will get detailed in depth video coverage of prostate
    operations etc etc etc etc..... yuk)

    Much Much more chance of being knocked off the bike and injured than
    getting cancer.

    I posted about the possibilty that cycling to an appointment to
    give a blood sample might lead to an elevated PSA reading leading to
    an unecessary prosate biopsy.

    If you've ever been subject to a totally innecessary prostate
    biopsy. you might not be so eager to flout your own ignorance
    in defence of your mate.

    But I can see you prefer to wallow in your own stupidity.

    Depends on your definition of injured. As about 1 in 4 people die of
    cancer (and proportionately more as they get older, and cyclists tend
    to live longer) For serious injuries, I'd reckon you'd be more likely
    to get cancer.

    Personally I find the obituaries column in Cycle morbidly comforting,
    as the average age and faculty level of the deceased is very high.

    It would 'appear' that there is no link between long distance saddle
    use and prostate cancer which is good enough for me at 55 this year.
    I will still have a chat with my consultant mate though (only problem
    is I know I will get detailed in depth video coverage of prostate
    operations etc etc etc etc..... yuk)

    Take you wellingtons and a waterproof pinney ;-) There is a link with
    testicular cancer though:

    "Coldman et al., in a case control study, investigated the relationship
    between the risk of testis cancer and various sports activities among
    133 men with testicular seminoma. After controlling for known risk
    factors such as cryptorchidism and after stratification for age, they
    found a significant increase in risk of testicular seminoma in men with
    history of regular bicycling (odds ratio 1.99), horse riding (odds ratio
    3.31) or both (odds ratio 4.56). They suggested that persistent
    repetitive trauma to the scrotal contents, as well as contact exposure
    to potential dyes and chemicals present in the leather saddles, may be
    the underlying cause in the development of testicular cancer in riders
    [53]. The relationship between exercise and the risk of testicular
    cancer was also addressed by Forman et al., who found that the risk of
    testicular cancer decreased with increased amount of exercise and
    increased with increased sedentary [54]. Therefore, although cycling
    per-se may be associated with increased risk of testicular cancer in a
    direct mechanism, the protective effect of exercise against testicular
    cancer may reduce this risk or cancel it completely [30]."

    Actually the other way round but the risk of getting cancer as a
    consequence of bike riding is pretty negligible
