Osteoarthritis ?

Nope. Changed from SPD to old toeclip pedals. Could be that all 3 left
pedals are tatered I suppose, they are all well used. I'm beginning to
wonder if the stem is loose/out of shape in the fork tube as it needed
a good bit of tightening up, but didn't make any difference to the
noise. Could have got pushed out of shape by being loose perhaps.

Yes but only by accident. Gave it a fair go but couldn't decide on
head or feet first. Neither seemed entirely satisfactory.

Assuming that you are standing so we can rule out the seat and seatpost
could it be frame flex? On a tight clearance frame it is possible for
the tyre to rub on the frame under too much effort.

Shoe? Stem/handlebarclamp, Or the downtube graunching on the
bottombracketshell, cracked frame?

Not sure what "graunching" describes exactly, but my most recent mystery
noise was cured by a good slathering of grease on the stem and on the
expander bolt.

Have at last isolated the cause - it was the left hand crank. Now
silent with an old crank in place. But still flummoxed - can't see how
or why it made a noise; it's been taken off and on, tightened and again
etc and there is absolutely nothing visible. Seems to be inherently
creaky but not badly fitted or anything.
It's a Shim. Deore Hollowtech standard on Dawes Galaxy and has done
about 7000km.

How about chain rubbing on cage of badly-adjusted front changer ?
