How do pumpusers bolus ?

    Before / During / After the meal ?

    In one go, in several goes or spread over
    15 / 30 / 60 minutes ?

    Which insuline ? (How much ?).

  2. meal bolusing is done just before a meal. my particular pump
    provides one and half units every minute.

    Taking a bolus after a meal is a "corrective bolus". usually
    used when one doesn't know the carb count of their meal and
    their blood sugar is higher than than had anticipated.

    Then there is the "pizza" bolusing method, or extended bolus
    will provide insulin over several hours to control the slow,
    but persistent bg rise from foods such as pizza and many
    Mexican foods.

    Let's start with the insulin. The vast majority of pumpers,
    ~94%, use a rapid acting insulin, Humalog or Novolog. The
    programing of the pump then determines how rapidly the bolus
    is injected. All pumps have flexibility in bolus delivery
    rates. In general, there is a regular bolus, an extended
    bolus, and a combination bolus. A regular bolus is delivered
    at a preprogrammed rate, usually over a minute or a few
    minutes. An extended bolus is delivered more slowly over an
    adjustable length of time, usually in 15 or 30 minute
    increments up to several to many hours. A combination bolus
    is, naturally, a combination of the two. Part of the insulin
    is delivered immediately and part is delivered slowly over
    time. The relative amounts and the extended timing are
    adjustable.

    These features allow you to adjust insulin delivery to match
    your particular response to the insulin taken, the meal
    characteristics (glycemic index, size, eating rate, etc.),
    pre meal blood glucose level, activity history and expected
    activity, and all the other things that affect post prandial
    blood glucose levels. These considerations, particularly pre
    prandial glucose, also affect whether you bolus before,
    during, or after the meal.

  4. Bolus? Well you should do this before you eat or if you
    forget after you eat. You can do it over any period of time.
    Insuline? Velosulin, Humalog, Novolog etc. Regualr Lilly
    works too.

    I was asking because I would like to know how pumpers do
    bolus, (not the available possibilities). Reason : Often I
    bolus after the meal, why ?

    Breakfast, it depends on my morning measurement. High is
    bolussing before the meal, low is after the meal. (Often
    spread over 15 minutes).

    Lunch, (almost all the time) after the meal, I am a bit hypo
    prone during the lunch. Often I have a little walk around
    lunch time. Two slides of bread, with 'content before the
    walk (30 minutes) but I do not bolus till after the walk.

    Diner, I should start the bolus when starting the meal, but
    bolus often after the meal, some time to long after the
    meal. I should dicipline myself more around this meal.
    Because off the amount of insuline for diner this is mostly
    spread over 15 minutes. If I am late I split the bolus in
    two parts one part at that moment the other part spread over
    15 minutes.

    I was wondering how others are doing this.

  6. Hi Ben,

    I often bolus after eating. My reasons are simple...I don't
    like to guess how much insulin I should take for an amount
    of food I have not yet eaten or been served, and I sometimes
    go low shortly after the meal, if I've bolused prior.

    I am very insulin-sensitive. Extremely minor adjustments in
    my basal rates produce large variances in my control. I
    mention this because your basal rates might be too high thus
    causing you to go low from bolusing prior to a meal. That
    seems to be the pattern I have found with my own control.

    That you've been told that you must bolus prior to a meal is
    merely the norm, it is by no means necessarily correct for
    everyone at all times. Do what works best for you.
