Erm, I don't think a beginner cyclist under expert tuition on borrowed
equipment should expect something so drastic to happen. I can only
think that some muppet put the pedal on the wrong crank, or the bike
wasn't fit to cycle. I'd have thought this was a very, well, near
impossible, thing to happen.

Certainly some muppetry involved somewhere!

I've had pedals loosen after the crank thread has been previously damaged
by cross-threading.

you're quite right this shouldn't happen but Having worked in the TV
industry I wonder if a lack of communication between the production team /
trainers, different working practices and time pressures (this kit *must* be
ready as the camera crew arrives at nnnn hours) led to safety checks being
skimped?

has led to a few nasty accidents in the past, most notably the chap falling
of the bungee rope in the Noel Edmonds show and Anthea Turner getting set on
fire....

The self-tightening nature of the threads is a bit of a cycling myth. My
wife has had a pedal come off from a display bike at a show, and it's
happened to me on my own bike when I only tightened the pedals by hand.
I always use a spanner these days - they don't need doing up
super-tight, but they do need doing up properly.

Which I'd expect if I was visiting a professional trainer and they are
providing the bikes.

These guys and gals are all novices in all the sports. It's a shame the
cycle equipment wasn't set up correctly. All the other sports seemed very
organised (of course, less to go wrong).
