you'll need to change everything.

I'm pretty sure you'll need a wider bottom bracket as well, either 113mm or 118mm.

Rear deraileur for double and triple set up are different. The rear STI should be OK, though.

The current Dura Ace STI shifters do not support triple rings (ST-7700/ST-7700(c)). The only one for triple is ST-7703. You will need to install the following items:
Front deraileur for triple (FD-7703)
Triple crankset (FC-7703)
Bottome bracket for triple (BB-7703)
Rear Deraileur for triple (RD-7700 GS)
STI shifter for triple (ST-7703)It's going to be an expensive upgrade to add a granny chain ring. I'd suggest you explore your options in terms of changing you current sprocket cassettte.

I put on a triple last summer - bought it as an upgrade to a Campy Chorus group. Items that were replaced:
Front shifter
Front der
BB (wider)
Crankset
Rear der (long cage)Long cage on the rear der is necessary, as the granny gear leaves too much slack in the chain for the road der to take up.
Correct STI shifters for DA triple is 7703 - I've been looking at them to put on a tandem. As far as I know, they only sell these in sets, not individually. I've noticed a lot of DA/Ultegra rear STI shifters on ebay - maybe left over from a triple upgrade?
It isn't cheap - upgrade to triple on a Campy group was an additional $200, while buying DA components new will probably cost you $400+. STI shifters alone are $200.
