Chronic diarrhea

  1. Hi,

    My mother (she's 51) told me today that she's had diarrhea for the past year!!! She doesn't want to
    go see a doctor because she is terrified of the result since her father (my grand-father) died of
    intestinal cancer 20 years ago. She also doesn't want to tell my father because she doesn't want to
    stress him out.

    First, what can I tell her or do to convince her to consult a physician? I told her that there are
    many different reasons possible to explain why she's been having diarrhea for the past year and
    cancer is only one of them, probably the least probable (hopefully) and that even if it were the
    case, there are treatments available that weren't 20 years ago. But she won't change her mind and
    refuses to see a doctor.

    Second, what do you think could cause this diarrhea? If it is cancer, what other possible symptoms
    should we be checking?

    Hi Claudia, It would be difficult to give you symptoms to look for because that would depend on the
    cause. I can't find one good comprehensive list of causes, but I do know two persons who had tumors
    (rectal and colon) who had diarrhea before they were diagnosed. (they've since passed away). Here's
    some websites and you'll have to make a list from these (and remove duplicates) and some for
    instance, parasites can encompass any number of types of parasites..same for food allergies (and
    these may not cover all of the possible causes but gets many of them).

    It must be mighty uncomfortable and many of the causes are treatable, so hope you can convince her
    to go talk to her doctor. It would probably start with having to collect two or three days worth of
    stool to check for fat malabsorption (or 2 or 3 samples/days for parasites). Then some other tests
    are mentioned depending on where other clues/symptoms might lead a doctor, to te point of a sigmoid
    or colonscopy, or small bowel follow through or barium test when they x-ray the barium going down to
    see if there's any masses etc. I sure don't know how to convince her, if you don't. If I think of an
    idea, I'll let you know.

    Let's just assume for the moment that it's not cancer, because I've got a couple of questions for
    your mother:

    Is there any sign of blood in the stools?

    If there is, your mother should get herself to a doctor for a colonoscopy because she might have
    ulcerative colitis or Crohn's disease, both of which are inflammatory bowel diseases. Neither of
    these can be cured, but they are both treatable and there are plenty of people of all ages living
    pretty nearly normal lives who just take some medication to alleviate the symptoms.

    If there's no blood, it's possible that she may have irritable bowel syndrome. Again, this can be
    treated; symptoms can be alleviated.

    My other question concerns diet:

    Is there something that your mother's eating or drinking that could be upsetting the normal balance
    and causing the diarrhoeia? Is she (for example) 'addicted' to prunes? It's not as silly as it
    sounds, BTW - an elderly lady I once knew was convinced that the dried fruit she was 'chain eating'
    was doing her good because it was 'cleaning her insides'. Well yes, but she was practically mal-
    nourished as well because everything was going straight through her.

    As for what to say to your mother - it's a toughie and you may have to be rather unpleasantly blunt
    with her. She's obviously worried, or she wouldn't have mentioned it to you, but burying her head in
    the sand isn't going to make 'it' go away. If she has got a serious medical problem, it's not going
    to make the slightest bit of difference whether she knows about it or not, so unless she really
    *wants* a slow, painful death, she might just as well find out so she can take appropriate action.
    Sooner or later your father is going to notice there's something wrong - and then *he's* not going
    to be particularly pleased that your mother didn't trust him, preferring to tell you and then
    telling you not to tell him. Well gosh, I can just imagine how impressed he's going to be. I've been
    through something similar, BTW - my mother has recently had surgery for cancer and although her
    husband (not my father) knew she was having "a cyst removed" he had no idea it was malignant. When
    she finally told him, he was very upset, the more so since I'd put two and two together and come up
    with the right diagnosis myself which he took to mean that mother had told me but not him.

    I don't know - families, eh? Oo'd 'ave 'em?

    You must convince your mother to see a doctor! Infections and chronic diseases have to be checked
    for. If nothing is found, I'd try folic acid. Diarrhea is one of the symptoms of folic acid
    deficiency. If your mother has had diarrhea over a year, she could also be deficient in some other
    nutrients, so it could be good idea to plan a comprehensive supplement program for her. Just have to
    be careful not to overdose magnesium and vitamin C. They are needed, but overdosing them could make
    diarrhea worse.

    Tell her she's more likely to die if she doesn't see a doctor than if she does. If it's really
    cancer (fairly unlikely) the sooner she starts treatment the better.
