But then I realised my flip-flop hub is 48 holes. Did some browsing for 48 hole rims but all I have found are BMX rims. Is that all a 48 hole hub is good for?

24 hole rim!🙂 i converted a 24 spoke rim and 32 hole hub into a 12 spoke front wheel. Just use half the spoke holes.

Nothing really but tandems ae pretty hard on wheels and a lot of 'em are built with 48 spokes - usually pretty thick spokes at that. You've got two people on the bike and usually a rear "drag brake" of some description (mostly hub brakes). More spokes (should) equal stronger wheels which given the forces that can be generated by HARD braking or two people pedalling is a "Good Thing(tm)".

Dave, get a 24H rim and use every second hole in the hub!!!!
