I asked a friend of mine, who works as a Perfumer, about the smell of Pear Drops. He said it smells like Acetone, and that this smell was used to diagnose Diabetes when he was young.
Acetone makes sense to me, I'm sure that is some form of byproduct of Ketoacidosis. But that's all the information I have,

It's how my cousins doctor dx'd her. Just walked into the house and said "Your baby's diabetic" and so she was. 18 months old at the time. That was 40 odd, maybe 50 years ago.

Most of the "tones" have a distinct aroma and we produce ketones when we're burning fat and muscke instead of glucose, so it makes sense to me too.

a fruity smell on the breath is commonly a sign of dka. confirm with a urine test and get to a doctor if moderate to high ketones are present.
