Is there an easy way to make it from just cocoa and sugar? My experience with powdered cocoa and iced coffee tells me that cocoa doesn't dissolve in cold liquids. Just heat the milk first? (but then I have to wait for it to get cold again...)

Yep. Make some hot cocoa and cool it down.
1/2 C sugar 2/4 C cocoa 3/3 C hot water 4 C milk
Mix everything except milk. Bring to a boil, then simmer a minute or two, whisking. Stir in milk, heat back up but don't boil.

Instead of adding the cocoa to the milk add the milk to the cocoa. Combine your cocoa and sugar thoroughly. Now add just a couple drops of milk and stir till it forms a paste. Slowly stir in the rest of the milk.

Easier than cocoa powder and sugar is some form of chocolate syrup. Fox's U-Bet or even Hershey's, whished rapidly into cold milk makes a pretty acceptable drink.

This may not really help you, but... we recently switched from using cocoa to 99% using unsweetened chocolate in making our 'chocolate sodas'. We dissolve a one ounce block of chocolate [we're using Scharffenberger] in about a cup of water by heating. You would of course use milk. For us the one ounce block is enough for a quart of 'soda'. Yes, there are a few flecks of solid chocolate in the final drink, but we think that that just makes it even more interesting. We cool the melted chocolate- liquid mix under cold running water, which takes just a few minutes. --thelma : -sw

I guess this is one of those things where I've forgotten the old-fashioned methods.
I'll be doing this tomorrow morning (so it'll be ready by the time I get my next craving). In the meantime, I made some chocolate frosting and spread it on chocolate graham crackers, with a side of milk for dunking and washing it down.

I tried that before I made the chocolate icing. It just ended up clumping up and not really dissolving. I could have tried the stick blender I guess.

It's easy enough to make good chocolate syrup. Just mix together approximately one part cocoa powder with three parts sugar and three parts water and a dash of salt. Bring it to a boil and boil it for a minute, then add a bit of vanilla extract.

Not at all. It's just that I don't have any, and it seesm unnecessary if it can be made from scratch.
Syrup is a convenience product. Chocolate milk, OTOH, has been around since the Aztecs. They didn't have Ovaltine back then.

Here's the final solution, combining several techniques mentioned in this thread.
Combine 1 tbls of cocoa and 3tbls sugar with 1.52oz water to make a slurry instead of a paste. Mix well and let sit for 3 minutes, mix again. Add 16ozs of milk (1 or 2%), stir again. Then I add ice cubes since I like it really cold. Didn't taste diluted at all.
This makes a prefect glass of chocolate milk - not thick like the commercial pre-made stuff, or when using the pre- made stuff or using the syrups or mixes with added thickeners (people really like thick milk?)
I'll try adding some fresh vanilla beans in the next batch - I'll probably mix up a large batch of this slurry and store it in the fridge. Better (and much cheaper) than any syrup or dry mix. Probably try and find some malt powder, too.

you may need just a pinch of salt in the water/cocoa/sugar syrup to really bring out the chocolate flavor
I usually only use 2 oz of water when making the chocolate syrup base for hot cocoa. Stir constantly.. the cocoa needs to cook a bit to lose "raw" flavor. Sort of the same concept in cooking roux (flour and fat used to thicken sauces/soups)

Chocolate milk, like 100,000 other recipes, was made at home from ingredients before someone put it in a container and sold it as a convenience food product.
