There will have been a pair of vaguely cross-shaped plates,
which are, of course, the bits that bolt to the (outside of
the) soles of the shoes. There will also have been four
bolts. There might also have been a pair of square-ish
plates, quite thin, with two parallel slots in them. These
are designed to go inside the shoe, below the insole. There
may also have been two sections with female-threaded holes.
These fit into the slotted plates I've just described,
inside the shoe. These parts may have been pre-fitted into
the shoe - in that case, you will have been able to see the
threaded aperture before you fitted the cleats initially.

They need to be reasonably tight, but no tighter than you
can easily manage by hand with a (normal length!) allen key.
They do sometimes come loose, but shouldn't do this
regularly. It's possible that the fittings in the shoes
"settled" a bit.
