Why not alternate days -- some days longer but slower,
others less time but a bit faster. Or do some days with the
same or a bit more time but include a couple of short (like
1-2 minutes) faster (just a tad) intervals.
Just don't increase both at the same time (same day) --
don't go both faster and longer in the same workout.
Conventional (i.e. for healthy folk) advice says don't
increase distance more than 10%/week. And if faster makes
you at all uncomfortable, slow down, and though you may not
have to end your workout don't be shy about doing so -- just
be more careful next time.
