What company made the part?

I bought it as part of the conversion, including stem, handlebar and headset. I gave it to the local bike shop to send back to the agents, so I unfortunately have no idea what make it is.
It snapped / sheared in the middle of the Cape Argus Cycle Tour last weekend. I managed to get it to tighten enough to finish, but steering was dodgy at best, and I couldn't stand on climbs and pull on the handlebars at all.
I was just curious to see if anyone has seen something like this happen before or not on a A-headset conversion from quill type fork steerer.
