Does anybody out there have experience with them? Are they comfortable?
I don't have any experience with Andiamo, but when it's extra cold out I wear a wicking brief that REI makes. It's not padded or anything, just a regular brief. But I'll bet it would make your dermatologist happy.

Does your dermatologist have any experience with cycling? Why does he think that underwear will prevent skin infections?
I've heard lots of recommendations on preventing saddle sores, but wearing underwear is not one of them. According to the roadbikerider.com web site (roadbikerider.comroadbikerider.comOpen ↗), "Most medical experts say that saddle sores are actually boils caused by skin bacteria that invade surface abrasions." The advantage of underwear in a multi-day ride is that you can carry a supply of clean ones. I am no dermatologist but I guess that clean underwear is healthier than dirty chamois.

Hmmmm...I don't know about that rationale. I do at least one week-long ride each year, but have never found a need to bring along cycling underwear. I typically bring along 2-4 pair of shorts and make do with that. When they all get dirty, I go to the laundromat. Also, to prevent them from "festering" before I can wash them, I turn them inside out after the ride, and put them on top of the tent for an hour or so...it allows them to dry out, and the sun's rays are a pretty good at killing bacteria (especially at the higher elevations where I ride in Colorado).

The last sentence is probably true enough. However, you can carry spare cycling shorts as easily as carrying cycling underwear. A small bottle of alcohol gel (yeeoooouuuuchhh!) to clean the groin if a shower isn't readily available can also help.
