Uncontrollable BP

  1. Would an 87 yr old with a BP of 214 despite all kinds of BP meds have any symptoms from a BP
    this high ?

    Are there symptoms/signs that will come and go ?

    Should they still be allowed to drive ?

    But as I understand it they have tried every medical avenue/drugs to control the BP and they dont
    work so if it was your patient what would you do ?

    I had 'uncontrollable' BP some months ago(180-200). I went to see 7 doctors with no avail. Looking
    back, if I had just added diurectics to an ACE inhibitor OR 24-hour slow releasing beta=blocker to a
    calcium channel blocker, my BP would have been controlled. But at that time, out of desperation and
    ...and probably a lot of ignorance on their part, my doctors prescribed a 'combination' approach for
    me. The latter includes a bit of everything. The drug interactions between those 'bits of
    everything' generated a lot of blood pressure spikes throughout the day. I took away those short-
    lived beta-blocker upon Dr. Chung's advice. Now, my BP is being normalized by 10 mg of Zestril,
    which could be strengthened with another tablet or 1/2 of a tablet of Diazide.

    Perhaps if you could tell Dr Chung what you are taking, he could give you some good ideas.

    I believe there is a saying about high blood pressure:

    "The first major symptom may be death"

    It's sometimes also called "the silent killer".

    So even if someone does not have any apparent adverse effects from high blood pressure that is not
    so say that they are not in danger.

    Have they also tried a combination of different drugs? One medicine is often not enough to control
    blood pressure. What does she currently take? IRC you mentioned that she still salts her food. This
    might be unadvisable and may reduce the effect of some medicines.

    I have never seen a patient whose blood pressure could not be controlled.

  8. Problem is we dont want her to drive but any suggestion that she might give up is met with hell fire
    She already takes BP meds plus diuretics has no intention of changing diet etc and the doctors seem
    to have tried all drug combos

    another tablet of hydrochloridediazide. They are separate tablets. The other doctor in HK used to
    prescribe me one single Lodoz tablet(diutectics and short-lived beta-blocker) and ask me to take
    that in conjunction with other pills such as Zestril or Nifedipine. The result was intermittent bp
    spike-ups, which the 7 other doctors called *my* 'psychological dependence'.

    If you had problems regarding medicine, Dr. Chung is the one you should go and the advice is FREE!

    I am not a doctor but by looking at the way your doctor prescribes to your grandmom, that doctor is
    not much of a doctor any way. I doubt if he is fully aware of drug interaction between all those
    kinds of drugs and what their impact is on her.

    I would, first of all, change a doctor, preferrably to one in a state hospital rather than a private
    clinic. After all, medicine prescription is more a function of experience than theories.

    Then you tell that doctor about 2 options suggested by very well known doctors:
    (1)10 mg of Zestril plus a tablet of Diazide(hydrochloridediazide) daily. OR
    (2)10 mg of Norvase plus 5 or 10 mg of 'Concord' 24 hours slow releasing beta-blocker.

    I would go for (1) first, in view of the fact that she is suffering from a swellen angle.

    But go for another doctor first and ask.
