I strongly suspect that swimming daily with pull buoys and paddles has produced this problem. I've tried to fix it by avoiding the pool and by just working with the kickboard, but it's still killing me.

That sounds like a nerve problem to me. The standard shoulder injuries hurt when you try to use the shoulder during swimming, not in the middle of the night. Swimming might be the cause, but I'm guessing it's a spinal problem in your neck.
