Near the end of a recent criterium style race I felt something strange was up with my bike - it felt like the headset had loosened a bit. When I inspected the bike it turned out that the rear wheel was loose - it appears that the QR release mechanism had loosend. I know it was snuggly tightened before the race. The frame is aluminium with aluminium vertical drop outs, the hub is a Hugii 240 and the skewers are branded DT swiss (they are the exposed cam type).
One side of the Hugii rear hub axle (the one without the cassette) doesn't have knurling and appearred to have worn a little bit..

one of the problems with exposed cam is that sometimes the pressure plate is not perfectly snug on tightening. if the pressure plate rotates to what should be the fully snug position during use, suddenly it's loose. closed cam is by far the best idea.

That seems to be the explanation - I've since changed to a shimano closed cam QR with not problems.
