During the course of the conversation I told her that I never wanted to exist in a vegetative state, dependent on some machine and taking fluids from a bottle.

Personally, I think that it should be a reverse onus, that heroic measures should not be made to save a person's life unless they have signed a form to acknowledge that they could end up in a chronic vegetative state.
I had an aunt who was living in a home. She had Alzheimer's and had had several strokes. She had signed a living will so there was a DNR order on her file. My father was her Power of Attorney after my uncle died, and my father made arrangements for me to take over the PoA when he was dying. About 4 months after my father died I got a phone call at about 3:30 on a Sunday morning. It was a doctor calling from a hospital to tell me that my aunt had had another stroke and he wanted to know if the DNR order still stood. I was feeling a little groggy, had to ask for a moment to clear my head. It was a heck of a time to make a life and death decision. But she had signed it when she was healthy and I figured that I had no option but to abide by her wishes. She died a week later.
