I do about 5 minutes of cardio before working out to warm up my
muscles. Sometimes I do 20 minutes of cardio after a workout or
    on another day.

Approximately 500/day or 3500/week above what you're eating now
    to gain 1 lbs.

Same answer. Protein and carbs have 4 calories/gram. Fat has 9
calories/gram. You don't want to gain a pound by eating fat.

Put together a workout that includes lifting weights and
moderate cardio and that should happen. My definition of
    moderate cardio is 20-40 minutes 3-4 times/week.

Start out doing a total body workout 3 times/week. Exercise
    examples: Upper body pushing: Bench Press, Dips, Military Press;
Upper body pulling: Rows, Lat Pulldowns; Legs: Squats,
Deadlifts, Leg Curls, Leg Extensions. Core exercises: Crunches,
    Situps, Stiff-Legged Deadlifts, Side Bends.

After you've done a full body workout for 6 months or so you can
start to split your days up into the different groups. Leg day,
    back day, core day, etc.

You do what's most important to you near the beginning of your workout.
If you're a runner, lift weights after running; if you're a lifter, lift
first then go out and jog.

If you're trying to add muscle, lift heavy, rest well, and eat hearty.
Google or buy the book on "20-rep squats" if you haven't already.

I assume you mean "running before or after" lifting? If so, it's a
personal preference. Personally after my 6 to 9 mile runs, I'm too
beat to lift, and doing it before helps my running form, so I prefer
    lifting then running.

BAHAHAHAHA! Spoken by a guy who "jogs" for 20 minutes, and thinks he's
    a runner.

There's no way your man-titted, saggy, pumpkin headed ass runs 6-9 miles.

FWIW, a well respected swimming coach in my area just switched that
protocol. In previous years, he'd have the swimmers lift after their swim
workouts (3-4 mile workouts). The problem was the guys were too fatigued to
use enough weight to make much in the way of strength gains. This year, he
switched to lifting first. The guys are definitely making more strength
gains - and for whatever reason, the lifting doesn't seem to adversely
effect the swim workouts as much as swimming adversely effected the lifting.

    I'm an ex-runner. My best 5k time was 20:10, which is 6:30/mile for 3.1
miles, at age 45. For a lot of years I used to do a pretty consistent
20-25 miles per week, once in a while upwards of 45 miles/week, and I've
done the Philly half-marathon 3 times although that was around age 30,
with a best of 1:36:40. My best 5k in those days was about 20:45 or so.
I gradually tired of long distance and found 5k training challenging and
rewarding so that's what I did in my early 40's.

Steve, how does one determine the amount of weight to use for 20-rep squats? Is
it a percent of the 1 rep max or is there some other "formula"?

They tell you in the book, which I don't have handy, but it's basically
a 10-rep max that you just tough out for 20 reps, which agrees with what
Keith said, just shy of what will make you deposit your lunch on the gym
floor. 3 breaths between reps. There are good (positive and
interesting to read as well) reviews of the book on amazon.com for
    anyone interested.

The actual title is "Super Squats," not "20 rep squats" - my bad. Also
plenty of info on 20-rep squat programs on the Internet if you don't
want to read the book, but it's inexpensive - $15, I think.
