Easy. You'll gain back very quickly. You can minimize atrophy by training the other arm (crazy, but true) and there is no reason you can't train the legs.

I'm sure they could use their arms to reach down and grip something to help hold the OP in position.

Rember that video someone posted of the Romanian, or other?, lifter doing the totally hands-free squats, including unracking and racking? Just do that.

You can also reduce atrophy by doing whatever mild exercises you can with the arm. Simply wriggling it a bit is a lot better than nothing. You can do whatever doesn't hurt.
