I ran off the road in the middle of a bike straightaway during this morning's triathlon because I was paying too much attention to my bottle cages and not enough to the shoulder of the road. Found myself angling off the berm when I finally stopped looking at the cages. Ended up riding 20mph+ through knee high grass until a hollow swallowed me up. Fortunately I escaped with only minor scrapes down right side and a bruised quad. and was able to get the bike and myself back into rideable condition after ten minutes of effort.
I've had close calls in the past but this is the first time I've actually gone over the edge.

I was getting drafted by this horrible fat girl once, and I told her to [censored], so she jumped on some passing fellow's wheel and drafted him. I was so angry, I put my head down and smashed it out... and looked up just as I was about to ram the back of a parked car on the course. I still don't know how I managed to jump a metre to the right to clear it.
Sigh, on a very long straight 2-lane highway, looked up there was only one cyclist way ahead... I'm pushing hard, head down on the tri-bars humming to myself, next thing I know my back wheel touches the inside of the cyclist ahead. We both come off on the rough grass verge... I was ok, she(oh yes for it was a woman) was a bit shaken up and her bike computer cable got all cuaght up and wrapped on hers drops(??)
We got everything sorted out, I apologised profusely and we both finished. She turned out to be the wife of a guy I knew... I sent flowers... never felt so dumb. And yes... I'm doing the Sand Key Triathlon at Clearwater at the weekend but have learned my lesson, honest!

I have hit a parked car once now, and was lucky no serious injury.
You just can't get too tired. Mine happened on the second day of too much riding, and too much work and stress the month before.

I read about a guy who was working full time, going to school full time, and Ironman training. He fell asleep during a descent on his aerobars and went off-road, but managed to hold it together. IIRC, he also fell asleep while swimming backstroke. He eventually realized maybe he couldn't pull it all off.

When writing up my thesis, and working many all-nighters, I often went to the pool to wake me up in the early morning. I would do 20 laps. One morning I woke up halfway down the depth of the pool, about 2 metres short of completing the 18th lap. Not good.

Someone at Cambridge, UK on tribars rode into the back of a parked car and broke his neck and died leaving wife and two little kids. So be safe out there.
