Ultrasound revealed bilateral duplicated renal systems, severe right upper pole hydronephrosis, and moderate left lower pole hydronephrosis. Computerized tomographic scan revealed a large, fecal-impacted colon. Diuretic renogram showed high-grade obstruction (T1/2 >30 minutes) of the right upper pole and left lower pole moieties. After administering an aggressive bowel preparation, a repeat diuretic renogram was performed, which revealed resolution of the left lower pole obstruction. Constipation has long been established as a contributing factor to urinary tract infections in children. However, fecal impaction causing ureteral obstruction in a child has not been reported. We report the case of an 8-year-old girl who had left lower pole ureteral obstruction secondary to fecal impaction. An endotracheal tube is always available in an operating room, so this technique is useful and effective in an emergent colon operation without special preparation. An 8-year-old girl presented with frequent, nonfebrile, urinary tract
infections and a lifelong history of severe constipation. Ultrasound revealed
bilateral duplicated renal systems, severe right upper pole hydronephrosis, and
moderate left lower pole hydronephrosis. Computerized tomographic scan revealed
a large, fecal-impacted colon. Diuretic renogram showed high-grade obstruction
(T1/2 >30 minutes) of the right upper pole and left lower pole moieties. After
administering an aggressive bowel preparation, a repeat diuretic renogram was
performed, which revealed resolution of the left lower pole obstruction.
Constipation has long been established as a contributing factor to urinary
tract infections in children. However, fecal impaction causing ureteral
obstruction in a child has not been reported. We report the case of an

no one "here" has ever said fecal impaction does not exist ....nor that it is
not healthy
so WHAT is the purpose of digging up an article that relates an incidence of
fecal impaction causing ureteral obstruction??
are you implying that this is a common outcome of constipation? it isn't of
course..

systems..ie two entirely separate renal systems?? hmmm perhaps the "fecal
impaction" was the RESULT of strange anatomy preventing normal bowel function??
nahhhh.....you have no clue what normal is anyway..so how would you know about
pathoanatomy as a cause in this case

this is a bit bizarre...never have heard of needing to use an "endotracheal
tube" to cleanse an impacted colon
certainly many many times those in ER settings and even in clinics..have donner
gloves and "disimpacted" folks...more commonly older folks...THEN used an oral
agent to completely clear the bowel..
I believe this article is describing a highly uncommon situation..where a
person needed "emergent colon" surgery..but the bowel was full of [censored]....
I say "uncommon" because it is not common to have [censored] expelled in the sterile
environment of an operating room...as it can guarantee almost a post op
infection...
again..Janny...continue to post articles that fail to support your "cause" when
you don't understand much of what you post anyway..more likely you just do a
word search...and post away..without reading..let alone comprehending the
content therein
at what point of your life will it ever sink in that you are totally out of
your very limited element...by arguing anatomy,,physiology..pathophys..etc with
a group where many many educated medical people read and post...
perhaps you could feel a bit more needed if you stuck to the "preschool
manager's" groups...

I would like to know what this child's diet consisted of. I see there is no
mention of her diet or exercise level. Was this yet another child on a
junk-food diet and getting no exercise?

I just joined this NG and am not sure what the issues are here regarding
chronic fecal impactions and severe constipation, but surely colonics are
not the answer. If the child had internal deformity that may be the cause
of her bowel problems. Most people would benefit more from a proper diet
and more exercise than resorting to colonics and other unnatural bowel
manipulations.
