Has anyone regeared a Twinn to put a hill climbing gear on
    it for real slow riding.

Not a Twinn, but I did regear an old RBC tandem for that purpose. I
replaced the original 14-23 [I think] 5-cog freewheel with an old 14-34
SunTour 5-cog freewheel, and replaced the original derailleur with a
SunTour V-GT, and it made a world of difference.
