I'm struggling with exactly the same problem (same sprocket), and my
der hanger is absolutely straight and the mech is brand new. Check the
width of sprocket-plus-spacer with a vernier caliper, it's possible
you've either got a manufacturing glitch or reassembled it slightly
awkwardly so one sprocket is oddly set up. I'm wondering whether the
spacers are specific to the sprockets so you have to get them in
specific orders or alignments, unlikely though that sounds.

The only other thing I can think of is the B-spring tension adjustment,
which I've yet to try on the problem bike. Perhaps moving the top
jockey wheel closer to or further away from the cogs might do the
trick.

If you solve yours I'd appreciate knowing how you managed...

Each click of the shifter pulls "x" amount of cable, and every "x" amount of
cable pull causes the der to move over a set amount. Make sure the der cable
is anchored exactly where it's supposed to be. That's one thing that can
screw up yer shifting.

But it's puzzling that it shifts all the other cogs ok.
