Dipyridamole's action as a vasodilator depends on an intact
(and healthy) endothelium. When there is coronary disease,
dipyridamole can cause either coronary "steal" and/or
"spasm" leading to regional ischemia.
Nitroglycerin is an endothelium-independent vasodilator
so that vasodilation will always occur. However, many
believe that it is the preload reduction that decreases
the workload of the heart that plays as important a
role in nitroglycerin's relieving angina as dilating
coronary arteries.
