Is it possible to get a new ultegra double with 53/42 chainrings, or is the 53/39 combo the only one available? Are there 42t inner rings that work well with ultegra setups that i could replace the 39t with?

You can use pretty much any inner ring as long as it fits the spider. The inner ring shouldn't need ramps or pins because it doesn't pick the chain up - it simply "falls" on the inner ring.n

It'll work. Did that with my TT bike, replaced the 39 with a 42.
