Three Speed "hub" in bb?

Has anyone seen the equivalent of a multi-speed hub placed into a
bottom bracket shell? In effect, replacing the axle with spindle, with
the chain leading back to a fixed cog or single freewheel?

Sounds like it would be hard to get a decent gear ratio, but maybe
with a Rohloff 10sp.

(Natch, the hub/bb would have a chainwheel attached.)
