STI levers worn?

It doesn't sound like a shifter problem. Is it a "middle" cog? Do you
have the problem when upshifting and downshifting?

I would double check the inner wire attachment to the derailleur. Make
sure it's routed as shown in the Shimano manual.

or more likely a cable problem.

Have swapped out cables including housings too... (sorry, forgot to mention
that)

Is it possible for an STI lever to wear ever-so-slightly (bearing in mind
the small tolerances involved in the design of this system I imagine we are
talking SMALL wear/damage to cause large effect) on just one index point??
FYI these levers are a few years old but have only covered max 3500 miles.

That isn't entirely out of the question. I had one just like yours,
it would shift poorly (just betw. 2 gears on cassette) even though I
replaced everything aside from the lever (including the frame, just to
eliminate the possibility of "frame flex"😉.

I finally gave up, replaced the lever, and have been happy every
since.

As a last ditch effort, make sure your last run of cable housing (the
segment between the rear derailleur and the chainstay) is sufficiently
long as recommended by our departed brother Sheldon Brown.
