Speedplay Cleat Came Out While Riding

Not sure about but I can recommend using shimano cleat.

I just bought Speedplay Zeros last week, this was my second ride with them. Midway through my ride the screws that hold in cleat gave way and came out and ended up lost in the rode. check your shoes as well I know a guy who yanked the screws as well as the part that attaches the screws to his carbon sole right out. those were look pedals though if it matters to you

The four cleat screws are coated with thread sealant. I've had the cleat screws loosen on the first ride or two after installation, but after one retorqueing they are always rock solid. Can't recall if Speedplay says to check the tightness after the first ride, but thats a good thing to do. Surprised you didn't notice the cleats coming loose before the screws fell out completely. An LBS Speedplay dealer should have extra screws and could help with the cleat installation. Could be the threads in your adapter base are stripped.....assume you're using an adapter plate and shims as interface to a three hole shoe mount.
