Some opinions from a retired engineer.
#3. Insulin Resistance is a function of body fat, exercise level, muscle
mass and genetics. I have never read any theories which considered grains and starches as
controlling factors.
Insulin Resistance is so important to you that you might consider it a "disease" separate from your
diabetes. It leads to premature heart attack. It is a strong contributor to the steady death of beta
cells which has caused you to become diabetic.
You fight it by:
a. Minimizing fat lb
b. Maximizing muscle lb
c. Exercising vigorously each day
d. Taking the anti-Insulin Resistance meds metformin, and (Actos or Avandia)
#4 As a rule of thumb, splitting your daily carb into as many small meals
as feasible is healthier for a diabetic. A specific answer to your question:
e. A larger mass of carb at any given meal will spike your bG higher.
f. As your bG rises, your body becomes more Insulin Resistant
g. Therefore, you must produce more insulin per gram of carb to handle the metabolism.
h. This is bad because higher levels of circulating insulin cause damage to arteries
i. This is bad because forcing your body to make more insulin results in more damage to
your beta cells which leads to a quicker deterioration in your "diabetes".
Think of your beta cells as something akin to flashlight batteries. The more you use them, the
faster they wear out, the sooner you will be totally dependant on "external power", or in this case,
multiple daily insulin injections.
#6. Your aerobic routine is wonderful, fantastic, life-extending,
diabetes-fighting,. . .ah. . . er. . .sorry, I ran out of superlatives.
j. Daily exercise reduces Insulin Resistance.
k. This makes your beta cells last longer ii. This protects your arteries
and thus makes your diabetic heart last longer
l. Daily exercise removes glucose from your blood without the need for insulin. It triggers
the AMP-activated protein kinase (AMPK) system. This is wonderful for diabetics. There is
reason to believe that, not even considering other benefits of exercise, boosting your
AMPK system (whatever on earth that is) reduces your heart attack risk.
Is your 40-minute routine sufficient? I have three answers:
m. Ask your genes (A snippy answer but unfortunately very accurate)
n. Take a look at your triglycerides to HDL ratio. If measured in mg/dL, a ratio of 3
represents a boundary between "too much" and "perhaps not too much" Insulin Resistance
(obviously a difficult subject to study). The lower the ratio, the lower your Insulin
Resistance.
o. Take an Insulin Resistance test. If you can score below 20 in the HOMA test, you are
"winning" . Professors T. Wiesner, M. Blüher and R. Paschke III of the Medical Department,
University of Leipzig, Germany assert that your target is 14.

Increased muscle mass is extraordinarily useful for all diabetics. If you can add resistance weight
training to your routine, you will be doing yourself a big favor. Muscles act like a giant "glucose
sink" which helps you normalize your blood sugars. That means lower bG spikes, and less
microvascular damage from high bG.
All in all, you are doing a superior job of attacking this disease. If you are really feeling
aggressive, consider a daily insulin shot as a beta cell protector as suggested by Professor Heyden
of the Medical School at the University of Missouri.
