  1. Where is this coming from? It only happens in one gear (middle crank and second to smallest rear cog), and only when I'm pedaling medium to fast.

    It is probably your chain rubbing against the front derailleur, I would think.

    I get this from time to time: the time I need to clean the cogs and chain. Try degreasing and relubricating. I use a product called "Krudcutter" to degrease and a motorcylce and farm equiptment spray lube to relube. It seems to last for about a month or so. Good luck.
