Make sure that your shifter is "shifted" (by pushing the
little lever in multiple times, and having tension on the
cable simultaneously) all the way. I've never had problems
removing cables from the shifters, frayed or otherwise.

Well, thanks to my collection of tiny little tools from my
model building days, I got the old cable out...

There's a small slot through that stupid lip. I risked a
fine screwdriver that acted as a wedge to lift the knob up
above the level of the lip. Then, using a pin vise to hold a
1/16" drill bit, I was eventually able to push it out (after
jabbing frayed cable ends into my finger tip more times than
I care to think about...)

It was really making me wish for the days of friction down
tube shifters and 6-spd blocks, except there's no way to
mount them on that frame.
