If you look at the table 1 that shows the subjects in the 1st and 5th quintiles, you can see some differences such as total calories consumed, the level of exercise, mean BMI, family history of diabetes, etc. Not only did the 5th quintile group consume more calories, they exercised less than the 1st quintile group. The 5th quintile had the highest risk for diabetes (table 4) as well as the highest family history of diabetes (table 1). The authors could have done more analysis with the information they had at hand.

Yep. Good points. I don't find this study all that convincing, frankly. For decades now the "conventional wisdom" has been that red meat and processed meats - especially stuff like bacon, hot dogs, Spam - are "bad for you". People who are somewhat knowlegable re nutritrition and motivated to eat well have been avoiding them. These same folks almost certainly have a lot of other good dietary habits that could well play a role in avoiding diabetes. They probably eat more vegetables and fruit, and of better quality and more likely to be fresh or lightly cooked. I'd guess they are more likely to take dietary supplements, etc. etc. As you point out , they appear to be more likely to exercise. There just seem to be too many variables, and certainly the majority of them do not appear to be controlled for.
