the video has to be on a undefined kind of video tape (...), and by
time of writing MiniDV didn't exist. But I assume any reasonable (not
too blurry) web-format video -might- do to, as long it leaves not any
doubt the execution was done correctly from start to end.
First you have to contact the IUF or USA to find out who is an
authorized examiner, and s/he probably can tell you.
You first may take a look at the 'list of clubs in the US'
(http://unicycling.org/usa/clubs/), and contact them.
