Transport for London has recently issued a new, copiously illustrated, edition of their bike facility Design Standards (that's why their bike facilities are so good)
TfL claims to have chosen random pictures of real London cyclists, without reference to whether or not the cyclists had helmets. I counted 29 cyclists with helmets, 52 without, and a few for which one couldn't tell.
