In order to justify "preventative" revascularization, a large amount of
hibernating myocardium would have to be demonstrated. In the absence of
symptoms of heart failure there would be no point in even trying to prove
this.

So, as far as I have understood, the angioplasty (in
the 100% occluded RCA and probably in the 70% LAD) was
recommended in order to 'secure' the heart from future
problems (of the LAD mainly), rather to relieve from any
present discomfort/problem...

With the additional information regarding the decreased EF, I would
lean more toward favoring revascularization for preserving/improving
EF.

Colin, clinically there can be evidence of heart failure sans symptoms.
