  1. Can anyone give me a nutshell explanation of why type 2 diabetics are more prone to weight issues?
    I am type 1 and have little experience with type 2. I'm in Nursing school and need some info on
    this subject.

    Its hard to say which comes first... excess Weight contributes to insulin resistance, which causes
    higher insulin production, insulin causes weight gain. Kind of a catch 22. Which started first is a
    toss-up. Insulin resistance or weight gain. It appears from much of the literature that weight is
    major contrubuting factor, though not the only one. Genetics also contribute.

    That said, insulin resistance does not make weight gain inevitable, only harder to control.

    Insulin does more than one thing. Insulin transports glucose from the blood stream, across the cell
    wall, into tissue. Insulin is also part of the hormonal control structure for metabolism.

    To get type 2 diabetes, you need to have insulin resistance and a defect in insulin production or
    form. The easiest, but certainly not the only, way to get insulin resistance, is to become obese.
    Most obese people are not diabetic, however, because there is a tremendous over capacity to produce
    insulin in the normal pancreas. They just keep pumping out insulin until it overcomes the insulin
    resistance and bg levels stay in their narrow normal range. If you don't have the normal over
    capacity, your bg levels rise and you are diagnosed as a diabetic.

    Notice that, in either case, you are producing high levels of insulin. The level of insulin, in
    relation to those of the counter regulatory hormones, has an effect on the metabolic system. Low
    levels of insulin stimulate the liver to release stored glucose into the bloodstream. High
    relative levels of insulin bias the energy storage pathways to store the energy from excess
    glucose more in fat.

    There seems to be a wide variation in the difficulty with which people lose weight. The
    hyperinsulinemia associated with insulin resistance is offered by some as part of the explanation.
    There are some popular weight loss schemes that are based on minimizing serum insulin excursions.

    That is the smallest nutshell of which I am capable. For further information do some searching on
    the terms 'metabolic syndrome' and 'syndrome x'.

    There are two main explanations which have been given; both seem to be supported.

    For those who have the genetic makeup needed for insulin resistance, excess weight increases the
    tendency for the insulin resistance to manifest itself.

    For those who have the genetic makeup, the hormones act to increase the tendency to obesity, as well
    as to bring on insulin resistance.

  5. Overweight is a risk factor for type II diabetes, but especially weight gain around the waist. Dr.
    Mirkin explains "Do you know why people who store fat primarily in their bellies are at high risk
    for heart attacks and diabetes? After you eat, blood sugar levels rise. If they rise too high, they
    call out huge amounts of insulin. Insulin closes arteries to cause heart attacks and acts on the
    brain to make you eat more and on your liver to manufacture more fat, and on the fat cells in your
    belly to store that fat.

    The liver removes insulin from the bloodstream, but insulin causes fat to be deposited in the liver
    which prevents the liver from removing insulin from the bloodstream, so people with fat bellies
    store too much fat in their livers, which prevents the livers from removing insulin, so insulin
    levels rise higher and higher to cause heart attacks, more abdominal obesity and eventually
    diabetes."
