  1. Note: According to this AP article new strains of viruses get their start in rural Asia. If that is
    accurate what factors account for it? Historically, it seems some of the worst viruses and
    diseases have come out of China. Although there is some dispute my general understanding is
    smallpox was brought to Europe from China. Does their overpopulation and cultural practices
    encourage a viral medium? I know in the case of SARS Chinese government officials were aware
    of this new potentially deadly virus and their response was a news black out. In this age of
    most likely emerging plagues the last thing called for is government secrecy. Getting crucial
    information out as soon as possible is more important than worries about tourism or investment
    being hurt.

    NEW YORK - As bad as this year's flu season is, it hasn't brought the worldwide outbreak known as a
    pandemic. But experts warn that a pandemic is coming, it's just a question of when.

    "It's going to happen," said Dr. Greg Poland of the Mayo Clinic. "For the American public in
    particular, I think it will be horrific."

    Many Americans haven't experienced the overwhelming crush of patients at hospitals and doctors'
    offices and the widespread fear a flu pandemic could bring. And by historical pattern, Poland said
    it's about time for the next one. There have been three in the past 100 years, igniting in 1918,
    1957 and 1968. There's no way to predict when the next one will appear, but the pattern does give
    experts pause. It's all up to a virus that is variable and fickle, constantly changing its genetic
    makeup, and the time when it hits upon a combination that lets it take off worldwide is a "roll of
    the genetic dice," said Dr. William Schaffner of Vanderbilt University.

    So the lack of a pandemic in the past 35 years basically means "the genetic dice haven't been rolled
    that way," Schaffner said. "While we're grateful for that, it makes us nervous."

    There's plenty to be nervous about. It's estimated that in the industrialized nations alone, the
    next pandemic is likely to send 1 million to 2.3 million people to the hospital and kill 280,000 to
    650,000, according to the World Health Organization. Its impact will probably be greatest in
    developing countries.

    As a practical matter, flu shots probably could not be counted on to prevent a pandemic. For one
    thing, pandemic virus strains emerge unexpectedly, and there would probably not be enough time to
    recognize the threat and then provide vaccines that target them, Schaffner said. What's more, many
    countries outside the United States wouldn't have the means to give enough flu shots to stop the
    spread, Poland said.

    Dr. Robert Couch of the Baylor College of Medicine noted that health authorities are making major
    efforts to prepare for controlling a pandemic, including putting an emphasis on developing and
    manufacturing vaccines faster and in greater quantities.

    The pandemic of 1918-19, known as the Spanish flu, sickened an estimated 20 percent to 40 percent of
    the worldwide population, with a death toll believed to exceed 20 million. In the United States
    alone, some 500,000 people died. An ordinary flu epidemic kills an average of 36,000 Americans.

    The next pandemic, the Asian flu of 1957-58, killed about 70,000 in the United States, while the 1968-
    69 Hong Kong flu led to about 34,000 deaths in the United States.

    New strains of the flu virus, and so potential pandemics, get their start in rural Asia, where the
    various strains that infect chickens and other birds, pigs and humans can mingle. That gives them a
    chance to swap genetic information as well as mutate on their own.

    The potential spark for a pandemic occurs when that environment produces a new virus that infects
    people and bears surface proteins that people's bodies have never seen before. That means people
    have no natural defense against it.

    In contrast, ordinary outbreaks like this year's come from a virus that has changed only slightly
    from previous ones, so that the population it enters still has some natural immunity from encounters
    with the previous germs.

    But the genetic shift alone is not enough to launch a pandemic. In addition, the new virus must
    acquire the ability to pass easily from person to person, either by random genetic change or by
    picking up genetic material from a previous human flu virus.

    The world has had some close calls in the past few years, says Richard Webby of St. Jude's
    Children's Research Hospital in Memphis. In 1997, a bird flu in Hong Kong jumped to people, killing
    six. But the virus never developed the ability to pass easily from person to person, Webby said.
    Hong Kong authorities slaughtered 1.4 million chickens to end the threat. Just this year,
    authorities became alarmed when a father and son in Hong Kong were hospitalized because of a bird
    flu virus, and when flu virus infected some workers in the Netherlands who had slaughtered infected
    chickens. The Netherlands outbreak was contained by anti-flu drugs and fast vaccination, and
    slaughter of the poultry, Webby said.

    Scientists have been noticing a lot of flu virus in chickens and pigs globally, and a lot of variety
    in the strains, which is worrisome, Webby said. It's impractical to develop vaccines against all the
    animal strains in case they jump to humans, and there's no reliable way to identify the most
    hazardous ones, he said.

    When the next pandemic shows up, experts say, it will find a population with many more vulnerable
    people like the elderly, infirm and those with weakened natural defenses than were living 35 years
    ago. It will also find a trimmed-down hospital system with fewer beds to handle a surge of patients.
    And while today's anti-flu drugs will probably attack the new strain, that's not yet clear. Supplies
    of the drugs and vaccines would be strained.

    But still, with the improvements in health care since the last pandemic, might the next one be
    less serious?

    "I want to believe that," Poland said, "but we won't know until it happens."

    animals, all of whom are susceptible to influenza infections, are placed in close contact with one
    another in high density concentrations in rural areas in Asia:

    China Is Perfect Breeding Ground for Viruses Like SARS, Expert Says Stefan Lovgren for National

    For a virulent virus seeking to jump from animals to humans China's Guangdong province may be the
    perfect habitat. Along the highways, ubiquitous farms are lined up next to each other, with farmers
    tending their ducks, chickens, and pigs in teeming and cramped quarters. In the city's food stalls,
    meanwhile, vendors keep their meat—alive and dead—in cages and baskets stacked on top of each other.
    Customers can choose from a menu of rats, cats, dogs, frogs, snakes, and exotic birds.

    Many wild and exotic animals are sold as food in China, such as these racoons in stacked cages at
    the Qingping Market in Guangzhou. Some scientists warn that such conditions are ideal for a virus to
    jump from animals to humans, which likely happened in the case of SARS.

    In such unhygienic conditions, scientists have long warned, it's just a matter of time before a
    hidden and potentially lethal virus would make the leap from animal to man. It should come as no
    surprise, then, that one virus—the enigmatic coronavirus that causes severe acute respiratory
    syndrome, or SARS—in all likelihood did.

    Among the first people to die from SARS, when the epidemic began in the southern Guangdong province
    late last year, were food handlers. Since then, at least 6,583 people in 30 countries have been
    infected with SARS. As of yesterday, the disease has claimed at least 461 lives.

    China has an unfortunate history of producing new viral strains. Two devastating influenza
    pandemics, in 1957 and 1968, both originated in China, each killing more than a million people.
    Experts maintain that outdated farming practices, overpopulation, and even political secrecy may
    be to blame.

    "China is the perfect breeding ground for new viruses," said Christoph Scholtissek, a German
    virologist whose research has linked China's traditional farming system with the emergence of lethal
    human viruses.

    Although researchers have—in record time—managed to crack the genetic make-up of SARS, the
    mysterious virus doesn't match any known animal or human viruses. In other words, scientists still
    don't know the source of SARS.

    Many emerging infections already exist in nature. They may enter humans as a result of changed
    ecological or environmental conditions that place humans in contact with previously inaccessible
    pathogens or the natural hosts that carry them.

    "The most likely scenario is that [SARS] has been circulating in another species in southern China,
    and human beings came in contact with it this past autumn," said Stephen Morse, an epidemiologist at
    Columbia University in New York and author of Emerging Viruses.

    Known as "zoonosis", the virus transfer from animals to humans has long been established. Foreign
    microbes can be lethal because the new host may not have the immunity that has built up over time in
    the original species. Some virologists believe traditional farming practices in China help spread
    new viruses. Chinese farmers raise ducks, pigs, and fish in one integrated system, and the animals
    may exchange viruses through their feces.

    Ducks can pass avian flu viruses to pigs, where two viruses can mingle and form a new strain that is
    passed on to the farmers. The pigs, which have a genetic make-up similar to humans, act as the
    "mixing vessels."

    Eurasian avian influenza viruses have been recognized as the precursors to the influenza virus genes
    that re-assorted with human strains to generate the 1957 and 1968 flu pandemics. However,
    Scholtissek, who linked China's agricultural practices with those influenza pandemics, said it's
    unlikely the same thing happened with SARS. While the flu virus' genome is divided into eight
    separate strands, which can break up and easily mix and match individual pieces to come up with
    brand new variants, the SARS virus consists of a solid genome.

    Escaping Detection

    But very little can be said with certainty about the SARS virus, partly because it's believed to be
    mutating all the time. Hong Kong scientists this weekend said the SARS virus is mutating rapidly
    into at least two forms.

    "This rapid evolution is like that of a murderer who's trying to change his fingerprints or even his
    appearance to escape detection," said Dennis Lo, a chemical pathologist at the Chinese University of
    Hong Kong.

    There is little doubt that crowded conditions, like those in China, play a major part in the spread
    of SARS. Shoppers in Guangdong province, which has a population of 76 million, throng the busy
    markets for fresh meat. The need to feed a growing population has greatly increased the number of
    animals raised.

    "Viruses transfer when you have a density of population of people and animals," said Ian Jones, a
    professor at the School of Animal and Microbial Sciences at the University of Reading in England.
    "Given that about a quarter of the world's population is living in China, things are just bound to
    pop up there."

    As people expand into ever-wilder areas, humans are coming in closer contact with wild animals. In
    southern China, wild and exotic animals like snakes and birds are treated as delicacies with many
    people believing they are good for one's health.

    Michael Lai, a professor at the University of Southern California in Los Angeles and an expert on
    coronaviruses, believes that SARS existed in a wild animal, most likely a wild bird, before
    transferring to humans.

    "The SARS viral sequence is different from any known human or animal," said Lai. "The sequence and
    genome structure of the SARS virus most closely resemble those of an avian coronavirus, [though] it
    also has a hint of a murine [or rodent] coronavirus."

    Secretive Government

    Add to the mix the Chinese government's capricious handling of the SARS outbreak. Although doctors
    in southern China were aware of the epidemic as early as January, information about the disease was
    kept secret by the government for months. It only began to emerge at the beginning of April, under
    intense international pressure.

    For months, doctors in places as far-flung as Canada scratched their heads as new cases of a
    mysterious pneumonia-like disease arrived at their hospitals. Health officials now charge that the
    epidemic could have been contained sooner if the Chinese government had been more forthcoming with
    information about the epidemic.

    In a rare public disclosure of failure, Chinese officials late last month admitted that infection
    rates were many times higher than initially reported, and the government fired both the health
    minister and the mayor of Beijing.

    I would add that one might blame it on the pigs. Avian flu is different enough from the human flu
    that it does not transmit directly to humnans but pigs can host both. When a pig has both types,
    pieces of the avian type can be transfered to the human type creating a completely new strain of
    human flu. The pig can then pass it on to a human. William L Hunt
