We have no idea how much if any running/walking/exercise that your
brother is doing/not doing.
If diabetes, there may be claudication of the arteries in his legs.
Poor circulatiion can be an issue contributing to the pain.
Remember that diabetes can be controlled and full active lives lived.
If one is not monitoring the issues that contribute to the pancreas not
being able to produce insulin then there are many problems that
develop.
With poor circulation, the problems of the healing of minor cuts,
blisters can lead to serious leg problems from major infections to
amputation.
Your comment "no major problems" raises the question: "How do you or
your brother or his other family members know there are no major
problems?"
