  1. I think I found the problem. I'm a little embarrassed but I didn't notice before that the axle is
    bent and one of the bearing surfaces is a bit rough.

    That will do it. Bent rear axles are very common with non-cassette-style wheels. If you ever upgrade
    to a cassette hub, it's unlikely you'll see a bent (and later broken) axle again.
