Simply put, I feel I will come out of my Sidi's heel first, when I start cranking on the cranks. Shoes are strapped on tight, and the fit it perfect, except for this heel issue. Any advice?

not sure which sidis you have, but make sure the first strap closest to the ankle is the tight one, since it limits how far forward your foot can go; you can make that one tight without discomfort, vs the strap(s) that run across you toe bones; make them too tight and it will hurt -
If your heel is narrow compared to your toes (a fairly common situation), you can put moleskin inserts in the heel of the shoe to take up the extra space. Most shoe stores and drug stores have these inserts.

Thanks Rick. I think that is going to be the plan at this point.
