assuming you are talking about handlebar shifters (either type) with the
adjuster on the down tube, that is just fine tuning AIUI. I think you just
have it in the granny ring with no slack and tighten it up on the front
mech. If it won't go into the big ring with the cable fully tight it could
be the limit on the mech, but unlikely if you have only replaced the cable.

Yes you can just buy the adjuster if it is the one on the frame tube. If it
is a flat bar type changer with the adjuster integral to that more of a
problem, but adjustment can be achieved on the clamp on the front mech.

Move the lever into its most relaxed position (smallest wheel). Hold
cage a bit out into its movement with either a finger or something to
wedge it or if necessary by mucking with the lower limit screw. Pull
cable as tight as you can and make fast. Then remover the finger or
wedgy thing or, if you've tampered with the limit screw, reset it.

This trick helps get the cable that little bit tighter, and should allow
you to get the full range of movement.
