Most don't. Originally, the cable left a downtube
lever, went under a guide, then pulled the cable
attachment arm from underneath. Now, if you're using
either Ergo or bar-ends, there ought to be a stop on
the downtube for cable housing.
Also back then, the only cable housing at all was between an
eyelet on the right chain stay and the rear derailleur -
same as now.
BTW, if you can get that for a dollar, grab it!

Problem is, it has no cable housing stop. What was
normally used in such a situation? (pictures of such a
setup would be *very* welcome)
Also, what are my options if I don't want to braze, drill,
or summon Moloch?
