  1. Did the normal 2.5 hour ride, which turns out to be 45.01 miles in
length, since today I refitted the bike computer because I wanted to see
    how fast I'm actually going.

However, today may not be representative, since I removed a shim from
the left shoe and put into the right shoe the orthotic I obtained today.

To my surprise, right at the start, without effort, in 39/14, I was
doing 20.1 on the flat. That boded well.

Right sole began complaining of course but I rode through it. For the
first hour I didn't do much more than 20. Second hour was mainly up, so
couldn't really tell; but the last half hour is on the flat on the way
home and there I comfortably - high level 3 effort - held a norm of 23
for ten miles or so, with decent stretches at 25 (which is 52/14).

I found that 52/14 gave a slower speed than I thought - a bit more than
    25, rather than 26/27.

The orthotic and the removal of the left shim led to a significant
improvement in comfort, apart from the right sole complaining. I could
feel my hips were level and the legs felt significantly more equal in
their motion. I also noticed in particular the lower right back was
free from tension - something I have until now been unable to achieve.

Did you have sore knees before the orthotic? I've not got one as yet, though
I use a heal raise in my normal shoes, also in the opposite foot. I
wondered how it would help cycling with the cleats, as the load is through
the front of the foot, though I expect just correcting the rotation of the
    ankle joint would work wonders.

Presumably its one orthotic that you move between shoes. Was there something
about interchangeable covers to allow washing of them?

    Good cadence.

    Slow cadence.

Are you pushing harder as you get faster? I use about 48x14 for 25mph which
gives roughly the same cadence as 39x14 for 20.1

Yes. That's why it takes time for me to get up to a higher operating
speed; the legs gradually increase the pressure with which they push.

Looks like I've been doing an Ullrich!

I'll experiment with the ratio/cadence, specifically with regard to
upping the cadence and changing ratio accordingly.
